Situations›Leaving care as a young adult
Starting adult life after care.
For young adults whose out-of-home care has ended, and for the people who have supported them. We support adults aged 18 and over.
How we approach it
Leaving care is a big change in a short time: a place to live, a routine, and a lot of services to deal with at once. We help with the practical parts and keep the support steady.

Leaving care
The first home, the first routine
Everyday livingPersonal care, help around the house, cooking, shopping and budgeting, at a pace that suits the person.
Getting around and being part of thingsGetting to appointments, work or study, building routines and getting out and about, with a worker alongside you.
An NDIS plan that fitsA plan written for a child may not have grown up with the person. We help review it and use it.
Getting started
How it starts
You don't need everything sorted. Send what you have.
Call us
Call us, or ask a case worker or support coordinator to.
We meet
We meet wherever the person is comfortable, talk about what they want their life to look like, and agree the first few things to do.
Support starts
You'll have a main contact from the first meeting. If something changes, you call them directly.
Questions
Things people ask us
Do you support people under 18?
We support adults aged 18 and over.
What if there's no NDIS plan yet?
To start funded supports, the person needs a plan or to be eligible for one. We'll talk you through what the NDIS looks for, and tell you if a Local Area Coordinator is the better first call.
What if the person has nowhere to live?
Call us as early as you can. Housing takes time, so the earlier we start the better.
Can a case worker refer?
Yes. Send what you have and flag the gaps.
