Situations›Coming out of hospital

Going home from hospital with support already in place.

For adults leaving a psychiatric unit, a medical ward or rehab, and for the social workers, case managers and families planning the discharge. We start before the day where we can, so there's someone ready when the person leaves.

How we approach it

Discharge dates can be set before anyone has lined up support at home. We talk with the person, their family and the ward team early, so the first day, the first week and the first month are planned.

Questions

Things people ask us

Can a hospital social worker or nurse refer?

Yes. Send what you have and flag the gaps. We won't contact the person until consent is in place.

What if there's no NDIS plan yet?

To start funded supports, the person needs a plan or to be eligible for one. We'll talk you through what the NDIS looks for, and tell you if a Local Area Coordinator is the better first call.

What if the person has nowhere to go?

Call us today, or ask the social worker to. Housing takes time, so the earlier we start the better.

Do you provide clinical care?

No. We don't provide psychiatry, psychology, nursing or allied health. We connect people to it and help keep it going.
